The obesity epidemic is fast increasing, this is a public health challenge; 97 million adults in United States are estimated to be overweight or obese. There are several co morbidities associated with obesity including hypertension, dyslipidaemia, type 2 diabetes, coronary artery disease, stroke, gallbladder disease, osteoarthritis, sleep apnoea , respiratory problems, cardiovascular disease(CVD) and diabetes mellitus.  Obesity also increases the risk of developing some cancers such as breast, prostate and colon cancer. Overweight and obesity increases the mortality rate which is the death rate, this can occur as a result of the co morbidities mentioned above. People who are obese or overweight may face stigmatisation and discrimination which might further exacerbate some of the problems mentioned above. Though, obesity and overweight are not frequently exclusive, since obese people are also overweight.  A BMI of 30 indicates an individual is about 30 pounds overweight; it may be exemplified by a 221-pound person who is 6 feet tall or a 186-pound individual who is 5 feet 6 inches tall. The BMI (Body Mass Index) is used to determine if an individual is overweight and/or obese, BMI takes into consideration the height to determine what category of weight an individual is. It is important to seek advice from the GP regarding what ideal weight you should be. The last decade has seen an increase in the percentage of adults who are overweight and obese. This is more apparent in individuals from a minority background, those on lower income and those not well educated. When an individual who is overweight and/or obese has other medical conditions present, the risk of developing co-morbidities mentioned above is increased

The overall quality of the diet plays an important role in health and poor dietary quality had been linked with an increased risk of unwanted weight gain in both men and women.
Inadequacy of a range of micronutrients appears to be linked with a higher risk of overweight and obesity and many individuals following popular diet plans have a high likelihood of becoming micronutrient deficient. Key micronutrients that may be deficient  in such plans include calcium, iron, potassium, folic acid, magnesium,  vitamin B12, thiamine, Vitamin D, vitamins A, C and E. Some of these micronutrients are already low in some population groups in the UK.
Easy steps to boost diet quality
1.    Consumption of a healthy balanced diet, including foods from all major food groups, in appropriate amounts; this remains the key advice to underpin optimal health and wellbeing.
2.    The voluntary addition of vitamins and minerals to foods by food manufacturers (also known as food fortification) has shown to be an effective food-based strategy to increase micronutrient intakes and reduce inadequacy- without increase calorie intake. Prevention of further weight gain is also an appropriate goal for people with a BMI of 25 to 29.9 who are not otherwise at high risk. A combination of diet modification, increased physical activity, and behaviour  therapy can be effective.

Dietary Therapy

Reductions of 500 to 1000 calories per day (kcal/day) will produce the recommended weight loss of 1 to 2 pounds a week as mentioned earlier. An effective dietary therapy should include guidelines for how to modify the diet appropriately for those individuals who wish to adopt a lifestyle of eating nutritionally balanced meals, taking into consideration total caloric intake. "Moderate caloric reduction is the goal for the majority of cases; however, diets with greater caloric deficits are used during active weight loss. The diet should be low in calories, but it should not be too low (less than 800 kcal/day). Diets lower than 800 kcal/day have been found to be no more effective than low-calorie diets in producing weight loss. They should not be used routinely, especially not by providers untrained in their use. In general, diets containing 1,000 to 1,200 kcal/day should be selected for most women; a diet between 1,200 kcal/day and 1,600 kcal/day should be chosen for men and may be appropriate for women who weigh 165 pounds or more, or who exercise". Long-term changes in food choices are more likely to be successful when an individual’s   preferences are taken into account and when the individual is educated about food composition, labeling, preparation, and portion size. Although dietary fat is a rich source of calories, reducing dietary fat without reducing calories will not produce weight loss. Calories are found in all foods and drinks (except plain water of course), yes even in alcohol! And these all contribute to weight. Frequent contact with practitioners during the period of diet adjustment is likely to improve compliance.

When an individual has made the decision to lose weight and ready to make the necessary lifestyle changes to achieve this, effective guidance is required to prevent any further weight gain). Because there is a greater risk of relapse and potentially greater weight gain later on. After the first 6 months of weight loss therapy, the priority should be weight maintenance achieved through combined changes in diet, physical activity, and behavior. Further weight loss can be considered after a period of effective weight maintenance.

Should I consider weight loss therapy?

Weight loss therapy is recommended for persons with a BMI ≥ 30 and for individuals with a BMI between 25 and 29.9 OR a high-risk waist circumference [which is > 35in (88 cm) in women or > 40in (102 cm) in men], and two or more risk factors (such as high blood pressure, high cholesterol, diabetes type 2, smoking etc). Remember to seek medical advice before starting any weight loss therapy.
